In-Home Services for frail seniors
Do you know someone who is medically fragile and is struggling to remain independent while living in their own home?
The Alternatives Program may be able to help by providing in-home services to medically frail adults who qualify and are at-risk for entering a nursing home.
The program's goal is to help keep frail adults in their homes by providing and coordinating a variety of services. The customer's needs are assessed by a social worker and a nurse who develop a care plan to address these needs. The program then contracts with private organizations, including home health agencies, to provide a wide variety of services such as home health aides, homemaking, emergency response systems and other services as needed. The program also provides Service Coordinators to assist seniors who live in residential sites for the elderly.
